Custom Form Design Services
Funded and non-funded grant projects may receive custom form consultation by Assessment and Testing. Consult with us before seeking outside assistance with your custom form design as we have several generic forms available that may serve as a less expensive alternative. Custom forms must be printed by an external vendor at the expense of the customer. Please note that custom form usage may be subject to fees.
When meeting with the Assessment and Testing staff to discuss the design of a new custom forms, please be prepared to provide the following information:
- Number of pages
- Number of timing lines per page
- Identify multiple answer fields (as opposed to mutually exclusive options)
- Indicate scoring specifications for exams: groups and keys, special calculations, scoring of sections, etc.
- Indicate any special requirements such as grouping that cannot be identified through data on the form, keeping forms in order, etc.
- Five blank samples of each form
- Five samples of each form containing test data
- One sample form containing notes that indicate the desired variable (field/zone) name and the values that should be associated with each option
